New Terms & Conditions of E-Payment Agreement
Dear Valued Distributors,
Kindly be informed that Terms & Conditions of E-Payment Agreement have been changed as follow effective immediately:
In consideration of CTOS Business System Sdn Bhd providing its services to the below name, please submit the following and fill the information below:
- Copy of Identification Card
- Consent Authorization Form
- E-Payment Agreement Form
- Terms & Conditions of E-Payment
After we have checked the applicant’s financial background, if approved, the applicant is required to submit the following:
- Payment slips for deposit and handling fees.
- Only accept Single Payment
- Photocopy of Standing Instruction form which is issued by CIMB Bank
- No other individual is allowed to pay for such agreement except for the applicant
- Applicant needs to provide the latest utility bill such as telephone bill/Astro bill/electrical bill/water bill/cukai pintu/cukai tanah/mobile phone bill which matches the mailing address
- If none of the mention utility bill above is under applicant’s name, applicant needs to fill up below section. Please specify the relationship with the person who is their name under the same mailing address with applicant.
- Address of the latest utility bill provided above must MATCH with applicant’s correspondence address for mailing purposes.
Note: All the documentations need to be filled up completely. Enagic reserves the right to reject such request should any of the documentation has not been filled up completely.
