Kindly be informed that our Product Application Forms; USER and DISTRIBUTOR have been changed as follow effectively on October 24th, 2016.
In consideration to the New Service Centre (Enagic Malaysia) in Kota Kinabalu, providing its services towards our distributors and users in East Malaysia, we have changed the ‘Shipping Details’ as below:
[vcex_spacing size=”30px”]We have included Alternate Payer Section into the Product Application Forms. The payer is required to fill up the Alternate Payer Section as an authorization statement of the payment made for the applicant. Alternate Payer Form is no longer required to be submitted along with the Product Application Forms.
[vcex_spacing size=”30px”]Each applicant may instruct a third party (collector) to collect the machine on his/her behalf. However, the applicant is required to fill up the Machine Pick-Up Authorization Section as a written consent for the machine to be collected by the third party. Do TAKE NOTE of the statement below as Enagic Malaysia will not be held responsible for any damage that is incurred upon the product while in transit by the time the machine has left the office. All liability shall be taken by the third party and the return policy shall take effect from the moment the machine has left the office.
